This program is designed for families in need of transitioning their loved ones from a nursing home,
Phase 1
(4.5 hours)
1.Initial consultation (usually over the phone) with client and/or family (30 min)
2.Gathering of appropriate information such as source of income, Medicare/Medicaid/Long Term Insurance status, and eligibility for available services and benefits in South Florida (30 min)
3.Over the phone or email contact with Social Worker, Case Manager and/or other healthcare professionals involved in the care of the client at the current location (1 hour)
4.Review of medical records and clinical documentation provided by the current facility (1 hour)
5.Discuss record analysis with current facility Social Worker/Case Manager to obtain medical clearance for travel and discharge options (30 min)
6.Preparation and delivery of report to family based on all gathered information and available options before entering Phase 2 of the program (1 hour)
Phase 2
This phase can only be initiated if client has medical clearance to travel and if qualification for benefits and services in South Florida is likely and family financial assistance is possible. This phase might take an unspecified period of time considering that not all facilities will take clients coming from out of the area or with benefit approval pending.
1.Search for facilities that are able to provide client’s level of care at the moment of the transition.
2.Contact identified facilities to verify client’s acceptance under their current benefits or pending approval of future benefits.
3.Coordinate tour of those facilities for family in order to decide which one meets their expectations.
4.Coordinate documentation for appropriate discharge, placement and transition.
5.Coordinate with family, client’s transportation arrangements. Provision of transportation and transportation fees are not included in the program fee.
6.Follow up with new facility after client’s arrival.
7.Follow up with family after client is admitted to new facility.
